Laser enucleation is increasingly becoming the standard of care for treatment of benign prostatic hyperplasia of all sizes.

چکیده
Laser technologies for benign prostatic hyperplasia (BPH) have steadily replaced transurethral resection of the prostate (TURP) over the past 10 yr in many parts of the world [1]. For example, the rate of TURP performance declined 47.6% in a US Medicare population between 2000 and 2008 [2]. Techniques for laser prostatectomy used in each country vary, with enucleation preferred in some countries [3] and vaporisation preferred in others [2]. The European Association of Urology and the American Urological Association BPH guidelines panels consider both holmium laser enucleation of the prostate (HoLEP) and photoselective vaporisation of the prostate to be equivalent to TURP, with long-term data favouring HoLEP; however, there are now a plethora of ‘‘me too’’ technologies (both electrosurgical and laser) that need to be considered, with a bewildering array of acronyms, now described [4]. The Thulium:Yttrium-Aluminium-Garnet (YAG) laser (2014 nm wavelength) is one of four contemporary groups of laser systems used for BPH treatment. Holmium:YAG (2140 nm wavelength), 532-nm lasers, and diode systems (a variety of wavelengths) are the others [4]. Thulium laser techniques have followed the development of holmium laser prostatectomy, which evolved from a modality used initially for ablation and resection to enucleation more than 15 years ago [5]. Currently, four distinct procedures utilise the thulium laser: vaporisation (ThuVAP), vaporesection (ThuVARP), vapoenucleation (ThuVEP), and enucleation (ThuLEP). Each is described by different authors [6]. In this edition of the journal, Gross and colleagues, who are pioneers with this wavelength, describe a series of 1080 patients who underwent ThuVEP at their institution [7]. They comprehensively document the periand early postoperative outcomes and complications in this

Laser prostate enucleation techniques.
INTRODUCTION Laser treatment of benign prostatic hyperplasia (BPH) through enucleation techniques has become increasingly more utilized in the field of urology. Laser enucleation of the prostate (LEP) is a transurethral procedure that employs several different types of lasers to dissect the adenoma from the surgical capsule in a retrograde fashion. MATERIALS AND METHODS We review basic laser ...
متن کاملComparison of Matrix Metalloproteinases 2 mRNA Expression in Prostatic Adenocarcinoma and Benign Prostatic Hyperplasia
Background and Aims: Prostate cancer is the second most common cancer in men worldwide in men. Matrix metalloproteinase 2 (MMP2) has a role in the invasion and destruction of the basement membrane and the extra-cellular matrix and facilitating the process of tumor cell invasion. The present study was conducted to compare the expression of MMP2 gene in prostate cancer (PCa) and benign prostatic ...
متن کاملEvolving Techniques for Surgical Treatment of Benign Prostatic Hyperplasia
The management of lower urinary tract symptoms due to benign prostatic hyperplasia (BPH) is one of the most topical areas in urology. Although most patients are adequately managed conservatively, many still require surgery to reduce bladder outlet obstruction or relieve symptoms by removing the inflamed adenomatous tissue. Transurethral resection of the prostate (TURP) remains the gold standard...
متن کاملTechnical Aspects of Holmium Laser Enucleation of the Prostate for Benign Prostatic Hyperplasia
Holmium laser enucleation of the prostate (HoLEP) is a minimally invasive procedure and a size-independent treatment for benign prostatic hyperplasia with excellent long-term surgical outcome. HoLEP has become an alternative to conventional transurethral resection of the prostate or open prostatectomy owing to its efficacy and safety. Although HoLEP is known to have a steep learning curve, very...
متن کاملA huge benign prostatic hyperplasia presenting with renal failure
Although transurethral resection of the prostate is still standard of care in many patients suffering from benign prostatic hyperplasia, traditional open prostatectomy (OP) seems as a widely applied method in larger glands. In spite of the fact that holmium laser enucleation can be performed in large glands, upper limits of prostate size in this method are not clearly identified in the current ...
